clang-format: tweak alignment settings

We already use 'PointerAlignment: Left'.  clang-13 and clang-14 added
'QualifierAlignment' and 'ReferenceAlignment' options.  Set those to
left similarly.

QualifierAlignment sets:
    * `const int` - right
    * `int const` - wrong

ReferenceAlignment sets:
    * `int& foo` - right
    * `int &foo` - wrong
