new diag and warm reboot targets

This commit supports the new warm-reboot, force-warm-reboot,
and diagnostic-mode targets.

Note that some systems, such as openpower based servers, will require
additional services from their corresponding meta layer to fully support
these new targets.

OpenBMC

meta-phosphor is the OpenBMC layer. This layer should be included for all OpenBMC systems. The OpenBMC layer contains content which is shared between all OpenBMC systems.
