README documentation update

Changes:
    - Remove reference of tests/ directory which
      is deprecated and removed in the latest
    - Remove REST username and password which is
      deprecated and removed in the latest
    - Replace tests/ directory reference with
      current redfish/ and ipmi/
    - Remove extended loop documentation which
      are no longer supported.
    - Re-organize documentation and cosmetic changes
